What causes upper back pain?

Pain in the upper and middle back can appear after a muscle strain and overuse, ligament injury and disc bulge or rupture. However, it can also occur as a consequence of poor posture while standing or sitting, after a physical strain or difficult exercise, uncomfortable sleeping position, etc.  In such cases, the pain develops slowly over time and is as such quite difficult to determine precisely.
